Generate an MIS report

Follow these steps to generate and send an MIS report.

Step 1. Find the company page

  1. Click "Groups" from the main navigation.
  2. Find and select the company.

Step 2. Generate the MIS report

  1. Click the "Reports" button and select MIS.
  2. Select the year and quarter for the MIS report and click "Generate."
  3. A report is generated that can be downloaded as a pdf or sent out as a fax and/or email. The contact information in the report is automatically filled in with the information you provided under the "notifications" link on the company page.

Step 3. Fix any errors (if necessary)

If any tests have issues you will see an error message at the top of the first page that says "There were some errors in the data. Please fix these so that we can produce an accurate report." Below that will be a list of links to each test that had an issue along with an explanation of the issue.

Here is a list of possible errors and how to fix each one:

  1. "DOT classified drug test is missing some of the DOT required 5-panel of tests."
    • This means that some of the substances in the standard DOT panel are not included in this particular test record.
    • To fix this: click on the test, take note of the positive substances (if any), click "actions" on top right and "change panel", choose "5-Substances (DOT)", click yes to confirm, then ensure that all substances are still correctly listed as positives.
  2. "Non-DOT classified substance has a non-negative result type."
    • This means that at least one of the substances in the test record is not part of the standard DOT panel. This may be because you are using a non-standard version of a particular substance (eg "AMP" instead of "Amphetamine").
    • To fix this: click on the test, take note of the positive substances (if any), click "actions" on top right and "change panel", choose "5-Substances (DOT)", click yes to confirm, then ensure that all substances are still correctly listed as positives.
  3. "DOT classified test has a Non-DOT result for 1 or more of its substances."
    • To fix this: click on the test, click on the "screen" tab on top left, update all substances to have standard DOT result values, and click save.
  4. "DOT classified test does not have a recognized DOT incident reason."
    • To fix this: click on the test, change the "reason" field to one of the standard DOT reasons listed on the MIS report, and click save
  5. "DOT classified test has a donor without a recognized DOT occupation."
    • To fix this: click on the test, find the "occupation" field on the top left section of the test record, type in the first few letters of the occupation name and it will auto-complete, and click save.
    • Note that you need to use one of the built-in DOT occupations that the system comes with, such as "Driver (FMCSA)" or Revenue Vehicle Operation (FTA)".
  6. "DOT classified alcohol test is not using a recognized DOT result."
    • To fix this: click on the test, update the "result" value on the top right to one of the standard values, and click save.
  7. "Alcohol test has no screen level or is non-number."
    • To fix this: click on the test, fill in the "screen level" value on the top right, and click save.
  8. "Alcohol test is unconfirmed for screen level greater than .02."
    • To fix this: click on the test, fill out the "confirmation level" value on the right side, and click save.
  9. "DOT classified drug test is not using a recognized DOT result."
    • To fix this: click on the test, update the "result" value on the top right to one of the standard values, and click save
  10.  Extra alcohol tests are showing up in "number of confirmation tests" column.
    • To fix this: click on the negative alcohol tests and ensure that no 0.0 level was entered in the confirmation level field if a confirmation test was not performed.
